Application for Mental Health Services

We want to make it as easy as possible for you to obtain the mental health services you need. This page seeks to provide an overview of the intake process, as well as some of the preliminary forms that you will need.

The first step to beginning services is what we call a screening. This gets the process started, gives us basic information about your situation. During the screening process, a clinican will ask you some questions about you and explain the intake process. We will answer your questions about treatment with us, and help you fill out the necessary paperwork. This screening is free of charge. We can do a screening with you over the phone, although if it is convenient for you to come into the office for a face-to-face screening the process is somewhat smoother.

After the initial screening, if you and the clinician decide to pursue treatment, an appointment for an assessment will be set with you. During the assessment, a clinician will gather information about you necessary to really understand what you are experiencing, establish diagnosis, and enable us to make informed recommendations for your treatment.

Once the assessment is complete, typically at the next appointment, you and your clinician will cooperatively develop a treatment plan based on the recommendations from the assessment, and treatment begins.

There are some forms that we can provide to you at our office. We also provide you with access to our privacy and treatment policies on this page. Our staff will also explain these to you as you get started with treatment.
